An employee in this position is responsible for transporting residents to and from appointments in a safe and courteous manner at all times.  New Hope Valley is a licensed Home for Aged (HFA) Assisted Living and Memory Care community.  An employee in this position works under the supervision of the Front Desk Manager. 




This is the primary driver for the Assisted Living Community, working with the elderly resident’s primary pickup and delivery to physicians’ offices and hospitals and dialysis, locally. 
1/ Be able to work with the elderly, managing a route that will accommodate and on-demand schedule.
2/ Act as a positive representative of New Hope Valley Assisted Living & Memory Care Community.  Work with the community using tact and understanding with nursing personnel as well as the caregivers and family members of the residents we serve.
3/ Secures passengers’ wheelchairs to retraining devices to stabilize wheelchairs during trip.  Be physically able to handle and assist the elderly resident in wheelchairs.  Assists residents into and out of the vehicle.
4/ Assist all other department in deliveries and pick-up and delivery of other related work items, i.e., x-rays, pharmaceuticals, mail, scripts, ect.
5/ Support medical records department in deliveries and pick up of medical records and physician orders.  All documents will be maintained within HIPPA guidelines.
6/ Communicates with New Hope Valley Assisted Living Community upon disruption in service.
7/ Cleans and services vehicle with fuel, lubricants, and accessories.  Report all vehicle repairs, and/or any unsafe or hazardous situations immediately to your supervisor.
8/ Be flexible and available for special occasions, and special schedules.
9/ Must consistently display and model the New Hope Valley Hospitality behavior model.
10/ Become familiar with location of community’s Disaster Manual and Material Safety Data Sheets (MSDS) and the reporting system specified in the Right to Know policy.
11/ Excellent attendance, timeliness in reporting to work, and discussion of all absences, injuries and illnesses with the Front Desk Manager.
12/ Attend minimum of 10 in-service training programs per 12 month period.
13/ Other related duties as assigned.

e) Display honesty, integrity, dependability and ability to work as a team member, possess good interpersonal skills and maintain a positive attitude toward employment; ability to communicate effectively and appropriately and maintain confidentiality of resident information in compliance with HIPPA regulations.
f) Reliable transportation to work with flexibility to work on any day of the week, any designated shift.
g) Ability to identify own personal strengths and weaknesses and discuss with supervision.
h) Satisfactory references, background check, drug screen, pre-employment physical and current PPD(or chest x-ray) for tuberculosis.



High School or GED equivalent
Previous work experience with van driving responsibilities



1/ Work is performed in a residential community setting; the presence of dirt, dust, fumes are inherent aspects of this position; high noise levels during performance of job may occur; possible exposure to chemicals and unpleasant conditions such as cleaning of human waste and bodily fluids to maintain a sanitary environment for residents, visitors and staff of the community.
2/ Speaking, hearing and seeing required to perform job responsibilities; reading/writing; understand directions and communicate verbally. 
Specific vision abilities required include color, close and far vision acuity, depth perception and ability to adjust vision focus.
3/ The employee must be in good physical health and have the ability to climb, balance, kneel, crouch, reach, walk-at least 3 miles/day, sit, lift, grasp, perform repetitive motions involving extensive bending/twisting-at least 25times/day, stooping, pushing, pulling, fingering, wrist motions and foot operations; ability to stand for an extended period of time without significant rest period; possess strength/stamina to lift more than 100lbs. with assist
