SUMMARY OF DUTIES:                                                                                                         


An employee in this position supports the Director of Resident Care (DRC) and ensures proper resident care is provided by the direct care partner to residents in a Home for Aged (HFA) Assisted Living and Memory Care environment and is responsible for monitoring each resident’s individualized service care plan.  An employee in this position reports directly to the Director of Resident Care.  This employee is responsible for leading the medication administration process and training, as well as other duties, as assigned by the Director of Resident Care.




1/ Direct and monitor resident care provided by the care staff to ensure each resident’s individual needs are consistently met and each resident’s service plan is being followed to provide the best possible care and living environment for the resident.  An employee in the position will be responsible for following up on changes in resident condition, assessing wounds, and initiating proper care directions to the care staff, while ensuring continuous follow up.  Proper follow-up will ensure that tasks are discontinued in a timely manner when issue is resolved.
2/ Completion of assessments for new and existing residents, requiring detailed documentation of resident history.  Assessments to be completed prior to admission and repeated annually or with a change in condition.
3/ Communicating new programs, procedures, interventions and/or changes in a resident’s health/circumstance to the care staff, and resident’s physician when indicated which includes coordinating necessary resident physician appointments.  
4/ Develop and maintain an open relationship with residents of the community through regular interaction, to encourage and support residents with their involvement in the community.
5/ Responsible for leading the medication management process including, medication documentation,

Ongoing training and review of med passers; overseeing documentation of controlled substance count and disposal according to state regulations; daily review of new orders and communication with residents, resident’s family, physician and pharmacy regarding medication orders and/or changes in resident’s medications.  Review of e-MAR and resolution of all flags via on-site review of deliveries and communication with pharmacy.  Responsible for monthly medication check-in of cycle medications and requested refills.

6/ Coordinate the request of prescriptions and ordering of diabetic supplies such as meters, test strips, safety lancets and retractable pen needles through local DME for all diabetic residents.
7/ Maintain a program of maintenance for all oxygen concentrators, nebulizers and CPAP machines.  This includes ensuring the machine is in good working order, filters are clean, cleaning tasks are scheduled in quick mar and supplies are reordered and changed when eligible through insurance.
8/ Meet with DRC to discuss coordination of services to residents, needs and challenges of resident care, including possible discharge; coordinate with DRC to confer with resident and/or resident’s family regarding changes in a resident’s status.
9/ Monitor documentation in and completion of the medication administration record; conduct resident chart audits to ensure accuracy and completeness of resident’s medical record.  Complete filing of all resident’s information in the resident chart.
10/ Oversee completion of regularly scheduled events regarding resident safety/health, e.g. resident vital signs, fire drills, resident TB testing, flu/pneumonia vaccinations.
11/ Practice appropriate lifting and transfer techniques when assisting residents; awareness of general safety, environmental, and infection control procedures; familiar with (PPE) Personal Protective Equipment and universal precautions relating to OSHA-Blood-borne Pathogen Standard.
12/ Knowledge of location of community’s Disaster Manual and Material Safety Data Sheets (MSDS) and the reporting system specified in the Right to Know policy.
13/ Excellent attendance, timeliness in reporting to work, and discussion of all absences, injuries and illnesses with DRC; attendance at professional seminars and community activities.
14/Attend minimum of 10 in-service training programs per 12-month period.





1/ Provide supervision, orientation and training to resident care staff, direct, monitor staff performance, assess workload, assign work responsibilities, review and monitor care staff attendance records and communicate any needs or concerns to DRC.
 2/ Assist DRC in the development and implementation of departmental policies, procedures and operations; accountable, dependable and assertive in identifying departmental and staffing needs.
3/ Assess problem situations to identify causes, process relevant information, generate possible solutions and make recommendations to resolve problem.
4/ Provide leadership and encourage a professional, positive attitude among community staff by being a role model in conduct, appearance, communication, mutual respect and ethical behavior and decision making. 
5/ Present an open, positive, and professional image to all residents, visitors, co-workers and general public by displaying general warmth and kindness in a friendly, courteous, respectful manner at all time and under all circumstances.
6/ Regular communication with DRC and input into community’s resident care program.
7/ Participate in planning and implementation of on-going quality assurance process and improvement efforts consistent with community’s philosophy and mission statement.




1/ Ability to perform necessary job responsibilities of Direct care partner and Medication Passer.
2/ Ability to manage care staff assigned to shift to insure all resident care needs are performed as well as resident calls to be answered in a timely manner. 
3/ Friendly, courteous manner; highly motivated, patient; excellent communication skills to direct resident care to range of residents with varying degrees of abilities.
4 Knowledge of assisted living and memory care principles, techniques, methods and current trends/developments regarding resident care including knowledge of Resident’s Rights, federal/state licensing and regulations for Home for Aged community.
5/ Knowledge of computer PC operations and software e.g. Microsoft Word, Excel, Outlook, etc.; good organizational and prioritization skills.
6/ Ability to be flexible, solve problems/conflicts effectively in a sensitive and calm manner, act independently and carry out oral and/or written instructions; ability to react quickly and decisively in the event of an emergency. 
7/Affinity for seniors and genuine care regarding their well-being with an ability to maintain appropriate attitude and conduct for the welfare of residents; compassionate; demonstrate an appreciation of the heritage, values, and wisdom of the resident and an understanding of their rights, the aging process and diseases of the elderly.
8/ Display honesty, integrity, dependability and ability to work as a team member, possess good leadership and interpersonal skills and maintain a positive attitude toward employment; able to communicate effectively and maintain confidentiality of resident and employee information in compliance with HIPPA regulations.
9/ Reliable transportation to work with flexibility to work on any day of the week, any designated shift.
10/ Ability to identify own personal strengths and weaknesses and discuss with supervision.
11/ Satisfactory references, background check, drug screen, pre-employment physical and current TB Test (or chest x-ray) for tuberculosis.




Minimum of 2 years of work experience as a Care Staff and Med Passer
Must be a Licensed Practical Nurse (LPN)




1/ Work is performed in a residential community setting; possible exposure to unpleasant conditions such as cleaning of human waste and bodily fluids to maintain a sanitary environment for residents, visitors and staff of the community.
2/ Speaking and hearing required to perform job responsibilities; reading/writing; understand directions and communicate verbally. 
3/ Specific vision abilities required include color, close and far vision acuity, depth perception and ability to adjust vision focus.
4/ Employee must be in good physical health and have the ability to climb, balance, kneel, crouch, reach, walk-at least 3 miles/day, sit, lift, grasp, perform repetitive motions involving extensive bending/twisting-at least 25 times/day, stooping, pushing, pulling, fingering, wrist motions and foot operations; ability to stand for an extended period of time without significant rest period; possess the strength/stamina to lift more than 100 lbs. with assist.
